The Community Health Aide Program (CHAP) Tribal Planning and Implementation (TPI) grant empowers Tribes and Tribal Organizations (T/TO) in the contiguous 48 states to plan and implement CHAP by providing regional flexibility, clinical support, tailored healthcare solutions, technological infrastructure, comprehensive training, integration strategies, and addressing social determinants of health.
