The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $3,200,000 for Year 1, subject to the availability of funds.The goal of this NOFO is to strengthen the capacity of the Government of Haiti (GOH) to improve health outcomes and reduce HIV related mortality and morbidity through the routine use of patient and health services data. The NOFO seeks to facilitate this goal by reengineering the health information system (HIS) to:Increase its use as a monitoring tool for continuity and quality of care.Strengthen public health data ecosystems for early detection of outbreaks.Improve the capacity to respond to complex health emergencies. Through this NOFO, you will improve the GOH's ability to monitor health outcomes including natality and mortality by improving the National Vital Registry system.You will work with the GOH to leverage and reinforce the built-in, innovative technologies of the existing electronic medical record (EMR) and information systems to Primary Health Care (PHC) and Vital Statistics. The final product should be a patient-centered integrated system able to support:Unique identification of patients.Longitudinal monitoring of acute and chronic illnesses.Continuum of care across all settings of the service delivery system (facility, community, and where applicable, border regions).Quality assurance programs.Coordinated disease monitoring activities and evidence-based responses for reportable conditions in case of disease outbreaks.Evaluation of the effectiveness of interventions and generation of reliable vital statistics.
